Primary care is often the first point of contact for people needing healthcare, usually provided by professionals such as GPs. From primary care, patients often move on to secondary care or other care settings.
In all care settings, it's vital to communicate with patients to ensure patient engagement and compliance. Incorporating digital automation into the process helps reduce the time spent in care coordination and encourages patients to participate actively in their care.
Our care coordination platform consists of a patient-facing mobile application and clinical dashboard, and it provides an effortless communication link between home and care teams. Patients receive all care-related instructions and questionnaires or forms through timed push-notifications, and care teams can monitor and manage patients' progress remotely.
Our platform ensures that patient data, such as forms and questionnaire collection, is done automatically. With the integrations, the data follows seamlessly from the GP practice to the specialised doctor and other ICS care settings.

This is Mary
Meet Mary, age 74, a diabetic with a debilitating hip, who uses Buddy Healthcare's Patient App.

Primary care
Her GP treats Mary’s diabetes in Primary care. Mary uses the Patient App, where she receives guidance that helps her manage her diabetes and submit forms/questionnaires to the practices’ diabetes team.


Secondary Care
When Mary goes to secondary care for hip surgery, she is supported with Buddy Healthcare's platform again. She is assigned to the hip surgery care pathway. Also, Mary’s primary care data follows her.

Social Care
Again, Mary needs additional services; she is assigned therapy at a mental health clinic, where she is boarded onto a psychiatric care pathway, supported by the Buddy Healthcare's platform.


Community Care
Mary requires further support with her care needs. Mary is assigned to a District Nurse team in the community. Her bespoke care pathway will help her manage her LTC and rehabilitate from her Hip surgery. Her health data follows her, throughout her care journey.
